Forum Dental is a an established and well-known dental practice that has been in operation for over 40 years. Our team consists of four general dentists, two hygienists, and three dental specialists - an Orthodontist, Periodontist, and an Oral Surgeon - who can meet the specific needs of nearly any patient. |

Dr. Andrew Cassidy DDS

Grew up in Lime Springs, Iowa where his father was a newspaper publisher for 62 years.

Attended the University of Iowa for both his B.S. (1975) and D.D.S. (1979) and then served in the United States Army Dental Corps for 21 years, retiring as a Colonel in 2000.

Board Certified by the American Board of General Dentistry and recognized as a Master of the Academy of General Dentistry.

Dr. Cassidy and his wife Cathy have two sons and are active members of the Rolla Church of Christ. They enjoy traveling and church-related dental mission trips to Africa and South America. |
Dr. Donald Richardson DMD

Born and raised in Oklahoma.

Attended the University of Oklahoma from 1966 to 1971 and then graduated with honors from Washington University's School of Medicine in 1975.

Served as a member of the U.S. Army Reserves from 1968 to 1996.

Member of the American Dental Association, Missouri Dental Association, Greater St. Louis Dental Society, Academy of General Dentistry, Lewis and Clark Study Club, and St. Louis Endodontic Study Club.

Dr. Richardson and his wife Cynthia live in West St. Louis County and have two children, Ann and John. |

Born in Chicago, Illinois.

Graduated from the Southern Illinois University School of Dental Medicine of Edwardsville, Illinois in 1979 and moved to Rolla immediately after.

Member of the American Dental Society, the Ozark Dental Society, and associate member of the Great St. Louis Dental Society.

Dr. Antolak has five children; Mindy, Brooke, Henry III, Autumn, and Thomas. He is happily married to his wife, Laura, and enjoys bowling, golf and triathlons. In fact, he completed his first half Ironman distance (1.2 mile swim, 56 mile bike, 13.2 mile run) in June 2006.

Dr. Antolak likes to do technically difficult cases, mostly involving cosmetic dentistry as well as seeing children as patients.
